Some healthcare professionals do not need a second English test
If you are being sponsored as a doctor, dentist, nurse or midwife and you have already passed an English language assessment accepted by the relevant UK professional regulator as a registration requirement, that assessment can meet the visa English requirement.
This is different from the normal visa test route. It means an assessment such as IELTS or OET may be usable through professional regulation even when it is not itself a Home Office SELT. The important point is that the relevant regulator accepted the assessment for your professional registration.
The same automatic shortcut is not stated for HCPC-regulated allied health professions. An allied health professional may still avoid a new test through nationality, qualifications, previous accepted visa evidence or another permitted route, but should not assume that HCPC registration alone settles the visa question.

Which healthcare professions can use a regulator-accepted English assessment?
Current Home Office guidance identifies doctors, dentists, nurses and midwives. The assessment must be one accepted as a requirement for registration by the relevant UK regulator.
Doctors
A doctor sponsored in an eligible medical occupation can meet the visa English requirement through an English language assessment accepted by the General Medical Council for registration.
Professional route statedNurses and midwives
A nurse or midwife can use an English language assessment accepted by the Nursing and Midwifery Council as a registration requirement, subject to the visa rule.

A dentist may meet the English requirement through an assessment accepted by the General Dental Council as part of professional registration.

HCPC professions are not named in the current automatic professional-assessment provision. Check whether another accepted evidence route applies before booking a SELT.

A professional may have registered through a qualification, nationality or another evidence pathway rather than by passing an English assessment. Check what evidence your regulator accepted and what your visa application asks you to provide.
When B2 applies and when the older B1 rule continues
From 8 January 2026, most new Skilled Worker and Health and Care Worker applications require CEFR B2 across reading, writing, speaking and listening. Transitional arrangements protect some existing visa holders.
New application
Most people making a new Health and Care Worker visa application need B2 unless another accepted evidence route or exemption applies.
Visa held before 8 January 2026
When extending or updating that Health and Care Worker visa, the transitional level remains B1 and English does not need to be proved again.
Switching from Skilled Worker
The current Health and Care Worker guidance says that a person switching from a Skilled Worker visa does not need to provide English evidence again.
Switching from another visa
A person switching from a different visa category normally needs to meet the B2 requirement.

Four checks before paying for another test
Keep copies of the official guidance and evidence used for your application. Provider names, test versions and visa rules can change.
Check your profession
Are you sponsored as a doctor, dentist, nurse or midwife and did you pass an assessment accepted by the relevant regulator?
Check previous visa evidence
Have you already proved English in a successful application, or do the pre-8 January 2026 transitional rules apply?
Check nationality and qualifications

Confirm the exact four-skills test, approved provider, location, B2 score and result validity.
The absence of an HCPC shortcut means you should check the normal evidence routes. It does not mean every physiotherapist, radiographer, occupational therapist, paramedic or other HCPC registrant must automatically book IELTS for UKVI.

The Health and Care Worker route requires reading, writing, speaking and listening. A provider being well known is not enough. The precise test and test location must appear on the current approved list.

The current approved-provider starting point includes Trinity College London, the IELTS SELT Consortium, LANGUAGECERT and Pearson. Verify the exact four-skills test and approved centre before booking.
Taking the test outside the UK
The current provider starting point includes PSI Services (UK) Ltd, the IELTS SELT Consortium, LANGUAGECERT and Pearson. Availability depends on the country and approved test locations.
All four skills
Use a test assessing reading, writing, speaking and listening, not a speaking-and-listening-only test.
Correct CEFR level
Most new applicants need to meet B2 in every required component when relying on the SELT route.
Result validity
Approved SELT results are normally valid for two years from the date the result is awarded.
Reference number
Use the unique reference number supplied by the approved provider in the visa application.
OET may meet the visa English requirement for a named healthcare profession when it has been accepted by the relevant professional regulator. Someone relying on the standard SELT route must choose a current Home Office-approved SELT instead.

Does OET count for a Health and Care Worker visa?
OET is not generally a Home Office SELT. However, doctors, dentists, nurses and midwives can meet the visa English requirement through an English assessment accepted by their relevant UK professional regulator. This is a separate route from taking a standard visa SELT.
Do nurses and midwives need IELTS for UKVI after passing NMC English requirements?

Does HCPC registration automatically meet the visa English requirement?
No automatic professional-regulation shortcut is stated for HCPC professions in the current Skilled Worker English guidance. An HCPC-regulated professional may still meet the requirement through nationality, an eligible qualification, previous accepted visa evidence or an approved four-skills SELT.
What English level is required for a new application in 2026?

Is standard IELTS the same as IELTS for UKVI?
No. Someone relying on the normal SELT route must use the exact approved UKVI test, provider and location. A standard IELTS result is not automatically a SELT. The professional-regulator route for certain healthcare professions is different.
